Making Physician Embedded Pharmacists
Although pharmacists know the value to a physician team, there should be ways to demonstrate it. In my opinion, providing an accessible roadmap on how to integrate pharmacists into physician practice is essential for a healthcare system that intends to adopt and implement a physician embedded pharmacists’ model. The roadmap begins with pharmacists earning the trust of physicians by establishing relationships with them (American Association of Coleges of Pharmacy). They can take an initiative of building up their patient panel to provide services on per day basis. After graduation, pharmacists can join physician organizations that implement the physician embedded pharmacist model in their practice.
To make it easy for pharmacists and students from colleges of pharmacy to adopt and implement the physician embedded model, they should sign up with innovative practice sites when they are their clerkship rotations (American Association of Coleges of Pharmacy). If they so that, it will be easy for them to reference this module so that they can gain insights regarding the practical steps they need to take, and the tools they find necessary to begin their practice. After receiving the necessary training and exposure to the model, I suggest that the newly graduated pharmacists should be able to replicate and scale this type of practice. They can then demonstrate their value in the physician practice by their ability to reconcile patient history with physicians’ notes, which is critical for them to run a community-based or ambulatory care.
Additionally, pharmacists can demonstrate their value in physician practice by helping to gather accurate medication and medical histories and collaborating with physicians to establish disease management plans (American Association of Coleges of Pharmacy). They can also educate patients about their disease, medications, and health in an effort to boost the physician’s prescriptions for the patient. Furthermore, I believe they can demonstrate their value in physician practice by using their experience to empower patients to utilize the knowledge and support they receive to take charge of their own health.
