Chairperson, Dr. Finch, members of the board, and all guests, good morning all? My name is Allan, and I hope we are all doing well this morning, aren’t we? I am delighted to have this opportunity to present a recommended solution to a problem we are all aware of; the medical errors that happen during the whole process of treating patients in our hospital. Medical errors threaten our patients’ overall safety and are a tremendous public health challenge. The sad part, ladies and gentlemen, is that all patients are equally vulnerable to this threat. It is for this reason, therefore, after thorough research, that I came up with a recommendation that aims at significantly reducing these medical errors. My recommendation is for us to adopt both an individual approach and system approach in handling medical errors, individual approach where healthcare workers are blamed for their forgetfulness or carelessness, and a systematic approach that involves cultivating better communication skills, adopting team working and embracing the evidence-based practice.

Adopting the individual approach and the system approach is exceptionally critical to keep at bay preventable medical errors. Blaming individuals for their forgetfulness or carelessness creates more alertness for medical practitioners when handling patients in the future. When healthcare workers are more alert and recurrence of errors are avoided, morbidity and mortality rates will decline significantly. Besides, blaming individuals in the affected patients’ presence makes such patients and their families emotionally satisfied.

The system approach focuses on the working conditions of individuals in organizations to avert medical errors. This approach is an essential strategy in the general performance of organizations. In team working, for example, when performing a surgery, an error of operation is avoided when several physicians are working together as a team. An error done by a surgeon will barely go unnoticed since the team members will raise the alarm, and hence the error will be corrected before it is a health concern to the patient. In a hospital setting during the whole treatment process, medical testing, prescription of drugs, and administering of drugs should be done by different practitioners to avert errors in using such drugs.

Excellent communication skills, written, verbal, and use of gestures in healthcare organizations are vital in minimizing avoidable medical errors. It improves interpersonal skills, and hence information is easily transferred from one individual to the next in a comprehensive manner. Communication skills will have a role in significantly getting rid of errors, such as failure to employ the indicated tests.

Adopting evidence-based practice as a system approach to averting medical errors involves acquiring quality education and research. Evidence-based practice is where results are obtained and decisions made based on facts rather than theory. This approach is essential in averting misdiagnosis errors and the use of outdated therapy.

However, let me say that on an individual approach, where individuals are blamed for carelessness or forgetfulness, medical errors are not usually made by the unfortunate few. It is often the best practitioners who make the worst mistakes, and such mistakes tend to fall on repeated patterns. Thus, medical errors cannot be eliminated because humans are fallible by nature, but it is possible to have minimal medical errors in healthcare.

In conclusion, let me retaliate that we should adopt both the individual approach and the system approach to minimize medical errors and mitigate the effects of such errors if we want to experience patients’ satisfaction in our healthcare facility. These approaches create more alertness among our healthcare workers, facilitate raising the alarm and earlier detection of errors and ensure smooth and adequate information flow amongst the doctors, surgeons, nurses, and all workers in the hospital. If we embrace and adopt these measures, we shall reap the full benefits of fulfilled practitioners and emotionally satisfied patients. Thank you.
